IMMIGRATION MEDICAL EXAMS

Dr. Natividad Verdejo-Perez is a designated civil surgeon authorized by the U.S. Citizenship and Immigration Services (USCIS) to complete medical examinations for a green card. 

 

Medical exams are performed on Thursdays. 

What To Bring To The Exam 

  1. Government-issued photo identification, such as a valid passport, Visa, or driver’s license. 

  2. Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, COVID-19, rotavirus and meningococcal disease

  3. Payment

 

During The Exam

  • The medical examination entails a review of your medical history and a physical examination.

  • The doctor will complete a comprehensive examination that will include a review of your medical history and a physical examination. 

  • The doctor will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea which will require blood and urine testing. If the applicant is found to have an infectious condition that is of a public health concern, it must be treated before the applicant can proceed to the next step in the immigration process. In addition, all applicants must have received all age appropriate vaccinations, as recommended by CDC (Centers for Disease Control). Orders for vaccinations will be provided if necessary.
